As part of the process for returning to 日韩无码 after taking a medical withdrawal, the university requires that you submit responses to the questions below at least 45 days in advance of the start of the applicable semester. Your Self-Report Letter must be submitted to the Office of the Vice Provost and Dean of Students either by email (please attach a Word or PDF doc) to deanofstudents@uvm.edu or can be faxed to 802-656-3467. 

Your letter must answer or address the following questions or prompts: 

  • Your name and Student ID number
  • Please describe the treatment you have received since leaving 日韩无码. 
  • Describe your plan for treatment when you return to 日韩无码.  
  • What supports/coping strategies will you have in place for your return?  
  • What makes you feel that you are ready to return to 日韩无码?  
  • What, if any, concerns do you have about returning to 日韩无码? 

Letter from Medical Provider(s)

In order to better support students returning to 日韩无码 after taking time away for medical reasons, we ask that a current treating medical provider respond to the questions below. Responses to these questions must appear on letterhead and should be submitted directly to the Office of the Vice Provost and Dean of Students by email to dos@uvm.edu or by fax to 802-656-3467.  For the purposes of this request, 鈥淧rovider鈥 means licensed healthcare provider (e.g., MD, DO, Psychologist, Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, etc.). This document must be received by the university no later than 45 days in advance of the student鈥檚 planned return.

Medical Providers, please submit a letter addressing the following prompts: 

  • The student鈥檚 name (and Student ID number if known). 
  • Treatment the student has received since leaving 日韩无码.
  • Your recommendations for on-going treatment and/or support when the student returns to 日韩无码.
  • What, if any, concerns do you have about the student returning to 日韩无码 and resuming a full course load. 
  • Any additional relevant information that could aid the university in supporting this student.

Alcohol Edu and Sexual Assault Prevention Online Courses

The online program AlcoholEdu was established in the summer of 2006 as a tangible population-level prevention program that challenges students' expectations around alcohol while encouraging students to make healthier and safer decisions. In cases of extreme intoxication/incapacitation due to alcohol and/or other drugs, it is imperative that someone calls for medical assistance. 日韩无码's Medical Amnesty Program serves to remove or reduce consequences faced by students in violation of 日韩无码's Code of Student Rights and Responsibilities to achieve the goal of student safety. Amnesty is granted from potential student conduct sanctions for themselves, the person in need of assistance, or the organization hosting the event where the incident occurs.
